Instructions for Completing the SRP Application

Returning participants are required to apply to both SRP and AADR.

For first-time participants, only the SRP application is required.

SRP applications are due by 12:00 noon on Monday, December 8, 2008 in the Office of Research.

Please follow the checklist below to assemble your application. If you are applying to the AADR, please use the AADR Instructions and checklist as well. Click on the links to access the required forms.

STUDENT RESEARCH PROGRAM APPLICATION CHECKLIST

Please assemble all documents in the following order. Do not staple! You must submit one (1) original and five (5) copies of the completed application.
  1. Student Research Program Application Face Page
  2. Citizenship Form
  3. Faculty Mentor's Statement of Commitment
  4. Applicant's curriculum vitae (not to exceed two pages)
  5. Faculty mentor's biographical sketch (NIH format, not to exceed two pages)
  6. Research Proposal (not to exceed 8 pages or 2800 words)
  7. Documentation of IRB approval or pending status for projects involving human subjects or vertebrate animals. Projects that use recombinant DNA must provide a statement that indicates conformity to NIH Guidelines for the use of Recombinant DNA.

DO NOT STAPLE ANYTHING! USE PAPERCLIPS AND/OR BINDER CLIPS!
