Your first visit to the Dental Faculty Associates will require extra time in order to complete your medical history and other forms.  Please arrive 15 minutes before your scheduled appointment.

 

If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk and present your insurance information to the receptionist. Patients under 18 years of age must be accompanied by a parent or guardian for their first visit.

 

We are located on the First Floor of the School of Dentistry in Room 1340 - adjacent to the Plaza Level of the patient parking area.
