Insurance Information

If you have insurance, we will assist you in receiving maximum benefits within your policy limits, including full utilization of dual insurance. Estimates of benefit coverage provided by us must still be verified by you with your insurance company. The School of Dentistry participates with many insurance companies; however, there are a few we do not participate with. For a list of companies we currently participate with, contact a Patient Services Assistant at the Information Desk. If we have a participating agreement with your insurance company, we will submit a claim on your behalf to your insurer once you have assigned your benefits to The University of Michigan School of Dentistry. However, if you receive payment from your insurance company, you are expected to send the payment to us within one week after you receive payment. As the patient you are always responsible for any charges not paid by your insurance.

 
If you are covered by Medicare, Medicaid, Champus, Workers Compensation or Automobile Accident Insurance or any government sponsored program, please discuss your payment situation with your clinic's financial counselor prior to treatment.
 
Due to many changes in insurance policies, it is no longer an easy task to interpret each individual policy. Although we try to stay aware of these changes, it is not always possible. It is your responsibility to know your individual coverage benefits and keep us informed. We suggest that you inform the scheduling clerk of any changes prior to setting up additional appointments. Failing to comply with this suggestion could result in you, the patient, being responsible for all costs incurred. Please remember your insurance policy is between you and your insurance company and not between the insurance company and your School of Dentistry provider.
 
 
Preauthorization or Predetermination of Benefits
Your clinic's financial counselor can send a request to your insurance company for any planned treatment to determine what your insurance will pay.
