First Visit - Registration
Your first visit will require extra time in order to complete your medical history, so please arrive 15 minutes earlier than your scheduled appointment.
If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk and present your insurance information to the receptionist. Patients under 18 years of age will need the signature of a parent or guardian on required forms.
Recall System
Our practice is focused on preventive care. Your primary dentist will recommend a preventive care program of routine check-ups to meet your individual oral health needs. For your convenience we have established an automatic recall system that will send you a reminder in the mail approximately two weeks prior to your next scheduled check-up apointment.
