Cancellation Policy
If you are unable to keep a clinic/surgery appointment please notify us at least 7 days in advance of the appointment. If our department is not notified within a timely manner it may affect patient care or the ability to reschedule an appointment. Not only is this courteous to our department, but allows other patients the ability to be seen sooner.
New Patient Visit
These visits are for evaluation only and do not in any way indicate a guarantee of care or insurance coverage.
Insurance
Once a request for surgery has been received from the attending physician, a prior authorization will be requested for the surgical procedures required, if necessary. A typical turnaround time from submitted request to scheduling of surgery is 30 days. This is adjusted based on medical urgency. Since this is a department that deals with urgent issues, an elective case will be secondary to life threatening cases.
In most cases you need a referral. To initiate a referral contact your primary care physician and have the referral coordinator issue an insurance referral and either fax it to our clinic or bring it with you at your clinic visit.
If you have an HMO health provider please allow at least 30 days to receive an insurance authorization for surgery. When we receive an authorization we will contact you to schedule a surgery date. If you receive an insurance approval/denial please notify our office below so that we may discuss an appropriate action.

- Aetna HMO
- Molina HMO
- United Health Care
- Health Alliance Plan
- Most out-of-state policies

Coverage of Surgical Procedures
Although our scheduling office will prior authorize medically necessary procedures, you are responsible for knowing your insurance and dental benefits including co-pays, deductibles, exclusions, and non-covered procedures.
Extractions are not expected to be covered by medical insurance.
Implants are not expected to be covered by medical insurance.
Elective surgery may not be covered and cosmetic surgery is not covered.
Day Before/Day of Surgery
Once you set up a surgery date please be aware that an arrival time will not be provided by our scheduling office as this is determined by the Pre-op Care Unit and will call you the day before surgery to notify you.
The operating room preoperative department calls patients 11am – 5pm to notify time of arrival as well as any necessary precautions or preoperative instructions. If you do not receive a call by 5pm the day before surgery you may contact the necessary departments below.

Healing
Healing is variable and will be typically best explained by the attending physician.
Follow Up Appointments
Return appointments following surgery will range in frequency from 2 weeks to several months depending on the condition and procedures performed. Return visits are typically covered for 90 days past a surgical operation as we do not charge for them in the clinic.
