New Patient Information

New patient exams are welcomed throughout the year. The first appointment may consist of an exam, cleaning and possible x-rays to determine your child’s dental needs. At the conclusion of that appointment, your child will be assigned to the most appropriate provider/clinic for his/her care based upon his/her individual needs.If your child has not been previously treated at the School of Dentistry, please plan to arrive 15-30 minutes prior to your appointment time to complete the registration process. To register, take your completed Medical History and Treatment Consent forms to the 2nd floor Information/Cashier desk, located in front of the main elevators. Once you have finished the registration process, you will bring the chart to the check-in window located in the Children’s Clinic

A parent or legal guardian must be present at the appointment for dental treatment to be performed. A student or resident will discuss the procedure planned for the initial visit. During the discussion of the planned treatment, you are encouraged to ask any questions regarding your child's dental health and planned care. Treatment can vary according to your child’s need and age. After the interview, your child will be escorted to the clinical area to be treated by the student/resident and faculty member. All of our faculty members are licensed pediatric dentists and have many years of knowledge and experience.

Fees and Payment Options

The parent/guardian accompanying the child is responsible for signing the child’s consent and treatment plan agreement form. By signing these forms the parent/guardian assumes the financial responsibility of the child’s treatment plan. We will bill most insurance companies as a courtesy to you; however, there are some insurance companies that are not part of our network and you will be required to pay for the services at the end of your appointment. Our Patient Business Office, located on the first floor, can assist you in printing off a courtesy claim for your insurance company’s reimbursement to you. You will want to contact your Human Resource Department to verify if the University of Michigan, School of Dentistry is listed as a participating provider for your insurance contract.
