Appointments
Note: All patients seen at the hospital must be referred by another physician or dentist.
In order to present the best service possible for our patients we ask that you follow these instructions before your appointment.
Patients new to the School of Dentistry will need to arrive 30 minutes early to complete the registration process which is handled at the 1st and 2nd floor information desks. If you are having IV sedation, you will need someone to help you get home after your surgery. Make arrangements for a friend or relative to drive you home the day of your surgery. They must remain in the waiting area during your surgery. There is a drinking fountain and restroom adjacent to the waiting room.
Insurance Information
Please bring any Medical/Dental history information and completed registration forms along with a written referral from you primary dentist. If you are already a patient at the school and a referral is in your chart, this step is not necessary. The referral should be written within 6 months of the time of service, signed, dated, and proposed treatment including listing tooth numbers. Extraction of wisdom teeth, multiple extractions of over 4 teeth or any IV sedation case requires a consultation appointment with us prior to the extraction. All consultations and emergency extraction appointments are scheduled for 9:00am or 2:00pm. Patients will be seen on a first come first serve basis. Patients should plan to be here either from 9:00am until noon or from 2:00pm until 5:00pm. Patients under the age of 18 must have a parent or legal guardian present.
Charges
Charges for any procedures, consultations, and X-rays are expected to be paid at the time of each visit. Fees vary depending on whether a student, resident, or faculty performs the surgery. All cases that require IV sedation are only performed by residents and faculty. The School of Dentistry will submit claims to participating insurance carriers and to Medicaid. If extensive treatment is needed, payment plans can be arranged with MED DIRECT LLC. at 1-866-575-6436 or 616-940-0500. Anesthesia, including IV sedation and nitrous oxide, are expected to be paid in full before the surgery along with 100% of the estimated surgery cost. All quotes for surgery are estimates and could change at the time of surgery. If your appointment requires a biopsy, the referral should come from your primary care physician. Biopsies are typically billed to your medical insurance.
Cancellation Policy
Our office has a strict appointment policy. Any cancellations must be phoned in 48 hours prior to your scheduled appointment time. If you do not notify our office in ample time, you may forfeit the chance to be rescheduled. Patients arriving over 15 minutes late to their scheduled appointment will be asked to reschedule.
