Applying to Dental Hygiene Graduate Program

1. Complete the Rackham Graduate School online application
2. Submit three letters of recommendation via Rackham application
3. Submit original dental hygiene program transcripts to: University of Michigan School of Dentistry, Office of Graduate Admissions Rm G226, 1011 N. University, Ann Arbor, MI 48109-1078
4. Foreign applicants whose native language is NOT English must submit TOEFL scores at
   www.ets.org - use institution code 1839
5. There is no specific deadline for this program, applications are accepted/reviewed at any time.
