Treatment for endodontics at the School of Dentistry is by "referral." Your dentist needs to write a letter of referral to us at:

The letter should explain to us the nature of your problem and contain a radiograph [x-ray] of your affected tooth.  Upon receipt of this information, we will contact you and set up an appointment for your endodontic treatment. If you have questions, you may contact our receptionist at 734-764-1540, but we will not be able to give you an appointment without your letter of referral.

Your first visit will require extra time in order to complete your medical history, so please arrive 15 minutes earlier than your scheduled appointment.

If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk [room 1310] and present your insurance information to the receptionist.  Patients under 18 years of age will need the signature of a parent or guardian on required forms.
