Congratulations to the Class of 2019!

Congratulations on being accepted the University of Michigan School of Dentistry!

Below you will find links to the paperwork you need to complete and return.  Please be mindful of the due dates.  If the School of Dentistry does not receive these signed documents by the indicated date, the assumption will be made that you no longer wish to enroll at the University of Michigan and another applicant will be offered the position. 

This is the first step in completing your educational journey.  In order to make this a smooth transition, there are four forms that needs to be completed.  In addition, there is relevant information regarding your matriculation.

All forms are due by January 20, 2015.

Once the forms are completed, there are three ways to return them to us.  They are listed in order of preference.

Personal Health Responsibilities
This document explains your responsibilities and risks in the dental profession. Return JUST the signed page (p. 2).

Technical Standards
This document explains the technical standards required to complete a dental education. Return JUST the signed pages (p. 5 & 6).

Record of Required Immunizations
This document records that you have the necessary immunizations. This must be completed and signed by your physician. Return JUST the signed page (p. 2).
