Congratulations to the Class of 2019!

Congratulations on your admission to the University of Michigan School of Dentistry!

All students who enroll in the DDS program at the School of Dentistry must comply with certain requirements including proof of immunization and technical standards for the practice of dentistry. Once you are offered admission, you will be asked to complete the forms/documents below (see Required Forms) and submit them to the Office of Admissions.

Deadlines for submission of the forms/documents are generally within one month of your admission offer.  If the School of Dentistry does not receive the signed documents by the deadline, the assumption will be made that you no longer wish to enroll at the University of Michigan and another candidates will be offered the position.

Once the forms/documents are completed, there are three ways to return them to us.  They are listed in order of preference.

Record of Required Immunizations
This document records that you have the necessary immunizations. This must be completed and signed by your physician. Return JUST the signed page (p. 2).
